Moving to a Total Triage system on 17th July 2025

Dear Patients,

The Evergreen Surgery would like to share some exciting news with you. We will be moving to total triage system from 17th July 2025. This means that all patients wanting to book an appointment will first be asked a short set of questions. This will be done online by filling out a simple form through Klinik.

If you are unable to use the online service, the reception team will be able to complete a Klinik form on your behalf.

The information you provide will help our GPs make sure you are seen by the most appropriate healthcare professional for your needs – which may not always be a GP.

After you’ve submitted your information, you won’t be given an appointment time immediately. Instead, your request will be reviewed by a GP, and we aim to get back to you on the same day with advice, a prescription, or an appointment, if needed.

This change marks an important step forward in how we deliver care and manage our services.

Total Triage is a new approach recommended by NHS England, to improve accessibility to Primary Care services. The main objectives are to enhance the quality of care our patients receive, to ensure that they are given the most appropriate appointment or advice to meet their needs and that this is done in a timely way.

We know that change can be difficult sometimes, but we are confident that Total Triage will bring many benefits to our patients – for example:

- Total Triage has been shown to reduce waiting times and it enables us to attend to your medical needs more promptly.

- Using digital communication will mean that you can engage with us from the comfort of your home or workplace.

- Triage ensures that limited healthcare resources are allocated to patients who need them most urgently.

- Total Triage will address the increasing demand for appointments and reduce the frustration of having to call and be in a long queue at 8am.
