Patient group – The Evergreen Surgery

We have an active patient participation group at our practice.

The Evergreen surgery has a local patient participation group that supports the surgery in ensuring that our patients have a voice in how the surgery works and services we offer. We are currently meeting quarterly online and annually in the surgery

If you are a registered patient with us and would like to be involved please ask the reception reception team team for a form or complete the online form below. We are particularly keen at the moment to encourage some younger members to the group and patients from BME backgrounds.
